Have you been hospitalized in the establishment and would like to obtain information about your hospitalization and/or your intervention?

Medical file

All information relating to your care and your state of health will be recorded in a medical file which can be computerized. It will be kept by the Silver Image private hospital for the life of the patient.

 

Communication of your hospitalization file

According to the Silver Image Healthcare rules, as a patient with parental authority, legal guardian or beneficiary of a deceased patient, you have a right access to the elements contained in the patient file directly or through a doctor. This consultation can take place either on site (free and the Silver Image private hospital offers you medical support, which you are not required to accept), or by sending photocopies to your address or that of a doctor that you have designated.

 

How to formulate your request?

The communication requesting communication of your file must be sent to the Vice President of Information Services of any Silver Image Hospital via email or post.

In order for it to be taken into account, your request must be accompanied by a copy of an official identity document (identity card, passport) as well as the reason for your request and the various required elements. In the event of an incomplete file, the establishment will return your entire administrative file to you with a letter indicating the missing documents.

Communication deadlines

Upon receipt of your complete administrative file, the establishment has 8 days to send you the desired information or q month if the medical information is more than 5 years old.

However, before taking these steps, it is advisable to contact the doctor who took care of you and who will be able to provide you with medical information concerning you. If you are a minor and you wish to keep your medical treatment confidential, you can object to the information collected during this treatment being communicated to your parents or to the person having parental authority. You can also demand that the consultation of medical information concerning you by your parents or by the holder of parental authority be carried out through a doctor.

 

Computer law and freedom

Each patient's file is computerized in accordance with the Information Technology Act 2000 in India. Each patient has a right of access and rectification to this file and can object to the computer processing of personal data concerning them. In the IT Act, 2000 in India, there are special provisions under Chapter III to grant legal recognition to electronic records, signature, and also encourage the government and its agencies to use them.
