Pre-admission
In order to welcome you in the best possible conditions on the day of your admission, you will be asked to go to the pre-admission service as soon as you have completed the consultation prior to your hospitalization with your surgeon or doctor.
The necessary parts
Before your stay at the Silver Image private hospital and your anesthesia consultation, you must pre-register with the pre-admission service. This step will make it easier to manage your admission. To do this, you must bring the following documents depending on your situation:
- the “request for admission” form given by your surgeon,
- the vital card and/or its certificate,
- the mutual card or electronic support for patients covered by a mutual insurance company with an internet portal,
- an ID,
- the estimate of fees from practitioners.
And optionally :
- the valid CMU (Universal Health Coverage) and/or AME (State Medical Aid) certificate,
- part 1 of work accident,
- for minor children, the family record book.
General conditions of hospitalization
During your pre-admission, the hostess informs you of the general conditions linked to your hospitalization: hotel supplements depending on the choice of services you wish to benefit from, the date and time of entry planned for your hospitalization, and entry formalities.
The pre-anesthesia consultation
Once the date of your hospitalization has been set by your surgeon, you must make an appointment with the anesthetist on 0825 74 34 44, choice 2.
This visit is mandatory for any surgical procedure. It must take place no later than 48 hours before your admission.
For this consultation, you must bring the prescription for the current treatments and your test results (ECG, heart ultrasound, etc.).
An adult or pediatric anesthesia questionnaire will be given to you to prepare for this consultation.
A coordinating nurse will receive you to explain the progress of your stay.
